Connecting the Community to Resources: An Update on Unite Us Santa Cruz County

When your patients or clients need resources or services beyond the scope of what you can offer, where do you send them? Once you do refer them elsewhere, what happens to them? Did they receive the services you hoped they would, or did they fall through the many cracks between multiple service systems and protocols?

Unite Us is a platform that aims to narrow and even eliminate these gaps by connecting service providers, clients, and data. Over the past several years, more Santa Cruz County agencies have joined the referral network—including Encompass Community Services, Salud Para La Gente, Community Bridges, Senior Network Services, Pájaro Valley Unified School District, and First 5 Santa Cruz County (just to name a few)—and more are getting ready to come onboard— including the Human Services Department and Health Services Agency, County Office of Education, United Way’s 2-1-1, and more!

How is Unite Us working for these organizations, now that they have had more time to become familiar with the platform? How could it work for your organization? Join us for a CORE Coffee Chat with Kaitlin Galanos, Unite California’s Community Engagement Manager, to explore these questions. Kaitlin will provide a brief overview and demonstration of how Unite Us works, and then we’ll have an opportunity to hear from local agencies about their experiences. We’ll also have an opportunity to explore your questions about Unite Us and the growing local network of participating agencies, whether your organization is already part of the network or would like to know more.
